Long Term Support Service Coordinator

This professional position is responsible for monitoring the long term care needs and funding for individuals with developmental disabilities who receive support services in Milwaukee County.  This position ensures that individuals have the appropriate levels of support services needed to live safely and as independently as possible.Minimum of 2 years experience in supporting people with disabilities.  Knowledge of Wisconsin's long term support funding sources, Medicaid Waivers Program documentation requirements and working knowledge of services systems for people with developmental disabilities is a plus but not a requirement.Must have a valid driver's license and reliable transportation.  Must have automobile insurance.  Must have acceptable driving record based on information from your MVR. A Criminal Background Check must meet the requirements of the State of WI licensure.Bachelor's degree in psychology, social welfare, sociology or a human service field preferred. Equal Opportunity Employer
