Medicaid Service Coordinator

The Brooklyn Bureau of Community Service is one of Brooklyn’s oldest and largest non-sectarian social services agencies.  The Brooklyn Bureau has been a leading provider of social services to families, children, and adults with disabilities since 1866.  The Brooklyn Bureau, which presently serves more than 10,000 individuals annually, is committed to a broad and diverse range of services including prevention of foster care placement; crisis intervention; early childhood and after school education; and vocational training, job placement, and clinical services to adults with physical and developmental disabilities, and histories of mental illness. 

The Brooklyn Bureau’s Medicaid Service Coordination provides assistance to people with developmental disabilities in gaining access to necessary services and supports appropriate to their needs and life goals. It is provided by qualified service coordinators and uses a person centered process to develop, implement and maintain an Individualized Service Plan (ISP).

 

Responsibilities:

Develop, implement, maintain, and oversee the Service Plans for a caseload of up to 30 MR/DD consumers.

Locate resources, programs, and other services for consumers.  Help consumers maintain eligibility for services.  Have monthly face-to-face meetings with consumers, including quarterly visits to their homes.

Respond to on call emergencies.

Maintain case records to meet audit standards.  Do intakes as required.  Liaise with DDSO office and other service providers to ensure coordination of services on behalf of consumers.

Hold family and advocacy meetings to ensure service satisfaction.

 

Requirements: Minimum educational level: A Bachelor’s degree in a health or human service field. Minimum experiential level: One-year experience working with people with a developmental disability, or one year experience working as a service coordinator with any population.  (With a Masters degree in a field of human services the minimum experiential level is not required.)

 

Other requirements: Good oral and written communication skills.  Must be skilled in handling emergencies and urgent matters while staying organized.  Must be flexible to do field work and be on call as needed.
