NURSE CONSULTANT - LONG TERM CARE OPTIONS
TITLE: NURSE CONSULTANT - LONG TERM CARE OPTIONS
DEFINITION: Reporting to an Assistant Director of Long Term Care Options (LTCO), the Registered Nurse Consultant (RNC) is responsible for providing team-based clinical consultation and consumer field visitation in the program that provides care management to younger disabled and chronically impaired older persons in their homes. The RNC position also involves nursing oversight of specified resource allocations, the development/maintenance of professional relationships with skilled service providers, and health-specific training/orientation of care management staff. In consultation with a designated Care Management Supervisor (CMS), the RNC helps facilitate the team’s delivery of quality services to consumers on a day-to-day basis.
QUALIFICATIONS:
Education/Professional Certification/Other:
§ RN/BSN in nursing required.
§ Current PA Registered Nurse license required.
§ Valid Driver’s License.
Professional Experience:
§ Minimum of two years experience in community-based service required, preferably in the field of aging.
§ Experience working with social workers and related disciplines is preferred.
Personal Characteristics:
§ High energy level, able to manage a variety of projects simultaneously.
§ Well developed interpersonal and communication skills.
§ Well organized and detail-oriented.
§ Commitment to quality consumer care
§ Second language abilities preferred.
DUTIES AND FUNCTIONS:
a. Under the supervision of the Assistant Director, carries out all such activities as assigned to meet the goals of the LTCO department within the context of the larger agency mission. Provides clinical consultation to a designated team and conducts consumer visits in field to help optimally allocate available resources and meet the community based, long term care needs of the disabled and chronically impaired older adults.
b. Assists the LTCO Management Team with planning/development initiatives specific to health-related aspects of the care management process/function. Make recommendations for improvements in health-related department-based policies and procedures.
c. Through ongoing field visitation, record/case review, team-based case conferences, and supervisory conferences with the Assistant Director, provides clinical consultation to care management staff and assists with the team’s adherence to department-based clinical protocols.
d. Utilizes client system to review clinical appropriateness of care planning process. Remains current with system changes/upgrades through ongoing computer training and orientation.
e. Assures that skilled service providers are complying with PCA contractual/care plan agreement and are in full compliance with state/federal standards and regulations applicable to LTCO, including Options and PDA Waiver requirements.
f. Monitors the delivery of services of contracted agencies, identifies patterns of problems with providers or barriers in service delivery systems and intervenes where necessary. Notifies supervisory staff in cases where supervisory/management intervention is required.
g. Assists with recruitment and hiring of care management and clerical support staff in conjunction with Human Resources and LTCO management staff.
h. Works with the LTCO Management Team to identify on-going training/orientation needs for care management staff in the area of health care delivery/monitoring and develops or arranges for training.
i. Participates in team-based case audits and reviews, and communicates findings for staff and program development purposes. Participates in department-based peer review process.
j. Works closely and collaboratively with other PCA Departments and staff to ensure that the goals of LTCO are being optimally met.
k. Meets various reporting requirements as required or otherwise deemed appropriate by the Director of Long Term Care Options. Assures accuracy/timeliness of reporting.
l. Based on availability, serves as a secondary back up for the team’s care management staff on a day-to-day basis, responding to questions and problems from providers, consumers and other department stakeholders.
m. Performs other duties as assigned.
