Outreach Nurse

Position Summary:

To provide quality delivery of health education, health screenings, and other services designed to meet the needs of the homeless population.

Assess homeless clients to ascertain feasibility of Comprehensive Care Program admission and patient’s desire/ability to gain employment, housing and increase their income.

Provide ongoing case management to nonresidential clients, connecting them to required services, mentoring their growth and independence.

Triage clients presenting to access outreach clinic services.

Assist Medical Practitioner with medical procedures as needed.

Provide direct hands-on nursing care as directed by Medical Practitioner (i.e. sterile dressings, injections, immunizations, etc.)

Document all nursing care rendered on progress note.

Provide transportation for non-residential clients in outreach van as needed.

Minimum Qualifications:

 

Must have a current valid Florida RN or LPN Nursing License in good standing with two years of nursing experience preferably in the community setting working with the economically disadvantaged.
