Description of Duties:

As part of a 7-person team, the Outreach Worker will be required to conduct outreach, mental health assessments, and follow up services to persons who are currently homeless and have a mental illness and also may have co-occurring substance abuse issues.  The candidate will also be required to work in team setting engaging clients on the streets, responding to referrals from shelters, family members, community etc… and linking clients to appropriate services.  Must be able to identify and intervene in crisis situations including hospitalizations. This team serves the south and west sides of Chicago.  Position requires timely submission of all documentation related to the delivery of services and effective advocacy skills.

Qualifications and Experience Required:

Master’s degree preferred, Bachelor’s degree in social work or other relevant field.  Applicants should be familiar with homeless issues and mental illness.
 
 
Requirements, Responsibilities and/or Duties Unique to the Position:

Valid Illinois Driver’s License, cellular phone (monthly reimbursement up to $50), computer proficiency. 

Minimum Education Required:  

B.A level required; Master's degree preferred.
