SUPPORTS COORDINATOR

The Supports Coordinator will provide services for individuals with mental retardation and their families.  The Supports Coordinator is primarily responsible for coordinating services based upon the development and implementation of an Individualized Service Plan (ISP) process for each eligible individual. This position reports directly to a Program Supervisor.

Responsibilities: Meet with new individuals, and their families as appropriate, within one week of assignment to explain mental retardation services, obtain consents and begin the development of an ISP Coordinate system specific required paperwork and documentation such as written consents and permissions, and procedural safeguards as required Make referrals to qualified support and service individuals and organizations and other appropriate resources to provide services as listed on each ISP within 24 hours of completing the ISPMake continuous referrals every 5 days until a provider for each service listed on an ISP is obtained and when providers for new services are needed Monitor the ISP with the individual at least quarterly but preferably once a month Facilitate the completion of an ISP review as required Document progress notes and monitoring screens weekly, at a minimum, from the time of assignment until all services on an ISP have started or if interruptions in services occur Collect all required data and complete all required HCSIS, MRS and PHMC documentation and reporting requirements Support and empower individuals to participate in their home and community and other support resourcesAdvocate for individuals and their families within and outside the mental retardation system Facilitate ISP, Reviews and Monitoring activities and meetings Attend meetings, trainings and conferences as required Complete other duties as assigned



Skills:Excellent verbal and written communication skillsExcellent interpersonal skills Ability to understand and follow policies and proceduresExcellent analytical and problem solving skills Good skills with typing on computer keyboards and electronic programsSpanish and other second languages, ASL and other cultural or adaptive areas are also preferred.

Experience:

A minimum of one year relevant work in supports/service coordination, case management and one years work experience in one or more of the following human services fields is required:working with individuals with physical, sensory and developmental disabilities; mental health or education. 

 

Education Requirement: At a minimum, Supports Coordinators should have earned a Bachelor's degree in an area of Human Services or Sociology, Political Science, Criminal Justice, Nursing, Counseling or Education and have a minimum of one year relevant work experience.

 

Salary:  Starting salaries vary and are based according to level of education, work experience and skills.
