Senior Re-Entry Counselor

Phoenix House, a non-profit organization, is the nation’s foremost provider of comprehensive, alcohol and substance abuse treatment for adults and adolescents. Since 1967, more than 100,000 drug-troubled individuals have received treatment at our adult residential centers, our adolescent Phoenix Academies, outpatient and day programs, and programs in prisons, drug courts, and homeless shelters. www.phoenixhouse.org

 SUMMARY

 

Under direct supervision of the Clinical Specialist, exercising independent judgment and discretion the Senior Re-Entry Counselor is responsible for facilitation of all re-entry and aftercare services.  This includes serving as primary care giver and role model to the residents of Phoenix Academy and to the community, and supporting the successful transition back to the community. 

 RESPONSIBILITIES

Supervises Re Entry Counselor II.

 

1. Facilitates Live-Out groups and records progress of clients in their charts.

 

2. Makes home, school and job-site visits to support and monitor the client in aftercare.

 

3. Prepares and presents seminars and workshops, creating opportunities for teaching general life skills such as cooking, cleaning living areas and maintaining clothes.

 

4. Facilitates encounter groups.

 

5. When assigned the responsibilities of Duty Staff, facilitates daily program activities.

 

6. Monitors and reports behaviors of clients.

 

7. Communicates rules and appropriate behaviors to children.

 

8. Disciplines children as appropriate.

 

9. Ensures safety of children

 

10. Role models appropriate behaviors to create a constructive home environment

 

11. Responds appropriately to incidents and problems that arise among the children in order to de-escalate problems as quickly as possible: uses crises intervention skills in communicating with children; assists with containments in accordance with Agency policies and practices.

 

12. Assists children in self-administering medications in accordance with prescriptions.

 

13. Establishes positive and nurturing relationships with children within professional boundaries and in accordance with Phoenix House policies and procedures.

 SECONDARY RESPONSIBILITIES

 

1. Monitors and records all re-entry and aftercare activities in the client's chart in a timely manner.

 

2. Participates in case conferences, Re-Entry Aftercare Meetings and staff meetings.

 

3. Approves all volunteer work, school plans and employment plans for Aftercare clients.

 

4. Participates in Quality Assurance and Utilization review committees as assigned.

 

5. Documents general information in the staff logs.

 

6. Maintains confidentiality over all records and information relative to residents.

 

7. Meets regularly with supervisor concerning resident and program issues; assists in development or modification of procedures to deal with individual children or groups.

 

8. Communicates with other staff with care or treatment responsibilities for children to review process, ensure consistency and continuity of individual treatment plans; and supports with unit's  work team.

 

9. May function as Facility Manager, coordinating daily staffing levels, visitors, residents’ day trips, conditions of facility are safe and secure, and general operations of the units.

Qualifications

Bachelor’s Degree in Social Services or Certification as Substance Abuse Counselor and experience working with adolescents in a mental health or substance abuse treatment program OR

Proof of enrollment in a college program and currently working towards a degree or certification plus related work expeience

Current and valid driver’s license

**Must meet federal, state and local Criminal Clearance and Child Abuse Indexing requirements.
