Social Work Case Manager

Grace Hill Neighborhood Health Centers, Inc. is currently accepting applications for the following position:

 

POSITION TITLE:    Social Work Case Manager / CDC (Children’s Development Center)

 

STATUS / HOURS:  Regular/Full-time (Evenings and weekends as needed)

 

STARTING SALARY RANGE:   $ 43,587.00 – $ 54,484.00 Annually

 

POSITION SUMMARY:  Coordinate the provision of case management services to CDC patients and their families. Integrate and coordinate care with their schools and other service providers.

 

EDUCATION REQUIRED:  Masters Degree in Social Work.

 

LICENSE(S)/CERTIFICATION(S) REQUIRED: 

 

1.  LCSW - Licensed Clinical Social Worker

2.  CPR Certification Required

 

EXPERIENCE REQUIRED:  

 

1.  Two (2) years experience in a health field setting as a Social Worker.

2.  One (1) year of experience working in an urban community.

3.  One (1) year of supervisory experience.
