SOCIAL WORKER - LMSW

GuildNet, a rapidly growing innovative long-term Medicaid Managed Care Plan for the chronically ill, disabled and visually impaired population throughout New York City seeks a Social Worker to coordinate and evaluate diverse health care and social services based on individual care plans.  Responsibilities include initiating and monitoring service authorizations and assisting members and families in adjusting to complex health and/or social issues.  Requirements include MSW, LMSW, two years experience working with the adult and elderly population, case management and health care services as well as, excellent communication skills and computer literacy.  Bilingual Chinese, Russian or Spanish skills a plus.  Positions available in White Plains and Manhattan.
