Description of Duties:
Assume the leadership role in the agency implied in the title of Team Leader.  The Team Leader is part of the program's management structure reporting to the Program Director or their designate.  The Team Leader has overall responsibility for the direct supervision of all other members of the team including but not limited to the management of team meetings to insure quality service delivery to Thresholds members, and effective interventions on their behalf.

Meet weekly with staff members to problem solve, train, monitor and guide their clinical case management and service delivery responsibilities toward effective, dignified and timely interventions.

Assume responsibility and oversight of intake process and service provision. Attend weekly meetings at Cermak Health Services at Cook County Jail to assess referred individuals for program eligibility and coordinate services. Responsible for oversight of ongoing linkage and community support services to members upon discharge from the jail. Responsible for communication with collaterals such as jail, court, and probation personnel as needed and desired by the consumer.

Provide direct service to members and their families, functioning as the senior clinician or "player coach" of your team.  This includes participation on the team in a manner consistent with team expectations and in a fashion which models the functioning expected of all team members.

Insure that the team functions optimally, in conjunction with other parts of the program while meeting the community support and rehabilitation needs of our members.  This is to be accomplished in a fashion which is consistent with the agency's mission statement, goals and objectives, formal and informal policies and procedures, as well as with the expectations and regulations of the various funding sources and with the highest professional standards.  This may include but is not limited to the standards and documentation requirements as established by: SAMHSA, CARF, Medicare, Medicaid, DMH, Social Security (PASS), and/ or DORS.

Qualifications Required:
Master’s required
Licensed Clinician, i.e. LCSW, LCPC preferred
Experienced in providing services to persons with serious mental illness preferred
Clear and effective communication skills both written and verbal
Functional Computer Skills
Knowledge of documentation required by CARF, Medicaid, and DHS

Requirements, Responsibilities and/or Duties Unique to the Position:
Valid Driver’s License, automobile and 100/300 auto liability insurance required

Minimum Education Required: Master’s degree required, licensed clinician preferred
