ED On Call Case Manager Department: Case Management Schedule: Pool Position Shift: Hours: M-F 4:30pm-8:00am, Sa/Su 8:00am-6:00pm or 6:00pm-8:00am Job Details:
The Emergency Department Medical Social Worker On-Call provides support to the ED for assessment and disposition of patients with identified needs including those related to mental health, AODA, abuse/neglect, domestic violence, in-home services, homelessness, or skilled nursing facility placement. Collaborates with physicians, and other health care providers to explore strategies, which will facilitate efficient throughput and discharge from the ED. The Medical Social Worker accomplishes this through an in-depth understanding of the components of healthcare delivery, which promotes timely, appropriate care, and positively impacts treatment delays or inappropriate resource utilization. This role actively provides utilization management discharge planning services for the organization.
Requirements:
- Master's Degree in Social Work from an accredited school of social work AND at least State of Wisconsin Social Work Advanced Practice Certification (CAPSW or LCSW), OR
- Master's Degree in appropriate field AND at least State of Wisconsin Social Work Independent Social Worker Certification (CISW, CAPSW or LCSW) - 2 years post graduate experience in a health care setting, preferably in medical case management
- ability to apply clinical social work theory and interventions
- ability to work independently, exercise independent judgment, establish priorities, and work efficiently and timely
- ability to establish and maintain positive, effective, collaborative relationships with patients, families, physicians, hospital staff and external providers
- ability to communicate well, both verbally and in writing, with a positive attitude and a commitment to
customer service and excellence
- ability for complex decision making and proactive, creative problem solving.
