Qualifications The Home Health Case Manager is the R.N. responsible for ensuring that patient care is coordinated and managed appropriately. The Home Health Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The Home Health Case Manager builds from the resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities. Registered nurse with current licensure to practice professional nursing in the State or as required by State regulations. CEUs or certifications must be current per State requirements. Experience in a health care setting, including 1 year experience in home health or as required by State regulations. Current CPR certification. Knowledge of philosophy, principles and practices of public health and its application to home health nursing. Ability to effectively deal with multidisciplinary health care providers. Excellent observation, verbal and written communication skills. Responsibilities 1. Provides nursing services to patients according to a written physicians plan of care. 2. Coordinates all direct and indirect patient services and care. 3. Assures compliance with local, State and Federal laws regarding licensure and certification of Agency and maintains compliance with accrediting body, if applicable. 4. Maintains department philosophy, goals and objectives, standards, policies and procedures and interprets these to ensure high-quality care. 5. Complies with acceptable professional standards and principles. 6. Communicates with the physician regarding patients needs and reports changes in the patients condition; obtains/receives physicians orders as required. 7. Conducts a comprehensive assessment of the patient. 8. In conjunction with the family and physician and/or authorized practitioner, develops and implements plan of care based on the assessment. Sets immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. 9. Provides appropriate skilled nursing care as outlined in plan of treatment to include: preventative and rehabilitative nursing procedures, nursing services, and treatment. Administers medications and treatments as prescribed by a physician. 10. Provides teaching and health care instructions to the patient/caregiver as appropriate to the assessment and the plan of care. 11. Conducts discharge planning for the patient. 12. Submits accurate, complete, timely, meaningful and required documentation. 13. Assumes on-call nurse role as necessary. 14. Reports all client complaints and incidents; implements appropriate corrective action as directed and follow-up when necessary. 15. Works with other departments to improve customer service, communication and care management. 16. Maintains productivity, as appropriate. 17. Participates in case conferences with the Clinical Manager by reviewing: services, clinical documentation and discussing pertinent patient care issues and goals. Implements suggestions of Clinical Manager to promote more effective performance and delivery of quality services. 18. Participates in quarterly record reviews and communicates findings and recommendations to appropriate Agency personnel. Participates in quality performance activities. 19. Submits an accurate record of visit/hours worked each day. 20. Assists with orientation of new staff as requested. 21. Attends in-service and continuing education programs. 22. Ensures timely and accurate communication and feedback to manager, physician and other individuals involved in the patients care. 23. Maintains and protects the confidentiality of patient information at all times, enforces patient rights regarding privacy, personal property, and grievances. 24. Instructs, supervises and evaluates home health aide care as required by Federal and/or State regulations. 25. Supervises LPNs according to Agency policy and State regulations. 26. Identifies patient and family/caregiver needs for other home health services and refers as necessary. 27. Communicates with referring physician and other Agency personnel to ensure coordinated and comprehensive care contributes to the total plan of care. 28. Assists with orientation of new staff as requested. 29. Attends in-service and continuing education programs. 30. Complies with acceptable professional standards and principles. 31. Works with other departments to improve customer service, communication and care management.
