Job Description

 

TITLE:                   LICENSED PRACTICAL NURSE II

 

DEPARTMENT:  RESIDENT CARE SERVICES

 

IMMEDIATE SUPERVISOR TITLE:               RESIDENT CARE SERVICES SUPERVISOR

 

 JOB SUMMARY

 

The licensed practical nurse provides basic and direct nursing care to residents promoting optimal independence and functioning as assigned by the registered nurse.  Performs a variety of technical procedures in the care of older adults.  In the absence of the Administrator, Assistant Administrator or Registered Nurse, acts as on-site charge person.

 Job Qualifications

 

Education:             Graduate of an accredited practical nurse training program.       

 

Experience:            None required.  Prefer one to two years experience with geriatric clients.

 

Licensure:              Currently licensed to practice in the State of Wisconsin.

 

Skills:                      Very good communication, data collection, organization, problem solving and decision making skills.

 

Physical and Mental Effort

 

Must be able to stand, walk, bend and squat frequently.  Must be able to lift, position and transfer residents occasionally.  Must possess levels of interpersonal skills in order to effectively interact with residents, families and personnel and to provide work direction and guidance to other staff.  Must possess a level of critical thinking skills necessary to determine need for intervention by a registered nurse or physician.  Ability to perform physical assessments within the scope and training of a licensed practical nurse.  Ability to provide input to the development of the Individualized Service Plan.

 

Environment and Working Conditions

 

Performs duties that may involve exposures to hazardous substances and is subject to normal risks.  May perform tasks which may expose the nurse to infectious or communicable disease.

 

Job Duties

 

The following description of job responsibilities and performance expectations is intended to reflect major responsibilities of the job, but is not intended to describe minor duties or other responsibilities as may be assigned from time to time.

 

RESPONSIBILITIES AND EXPECTATIONS

 

1.                    Demonstrates a commitment to the Mission and Philosophy of Villa St. Francis to preserve the dignity and worth of every person.

 

                Demonstrates courteous behavior through polite and respectful communications with residents/families, co-workers and visitors.

 

            Ensures each resident’s right to privacy, confidentiality at all times.

 

            Establishes a therapeutic and trusting relationship with the residents which ensures them a safe environment and the protection of their rights.

 

2.                    Communicates verbally and in writing all pertinent resident information to assist in developing and disseminating the resident’s service plan to all the resident care staff.

 

            Documents on an on-going basis, changes in resident’s condition and service plan according to policy (nurses’ notes, communication book, PCA notes, etc.).

 

            Collaborates with the registered nurse in planning services to meet resident’s needs (hospice, therapy, scheduled tests, medical supplies, etc.)

 

            Shares pertinent information with members of the resident care team to ensure continuity of care (shift report, communication book, PCA notes, etc.).

 

2.4   Reviews and updates all medication records each month.

 

2.5   Coordinates and documents all lab orders each month.

 

3.                    The Licensed Practical Nurse, based upon preparation and experience, provides direct nursing care in accordance with nursing process.

 

3.1   Delivers care according to established policies and procedures, the Individualized Service Plan and Physician Orders.

 

3.2     Collects and documents data and information regarding residents’ health status.

 

3.3     Recognizes symptoms and/or deviations from previous data and communicates changes in condition appropriately.

 

3.4     Maintains a safe environment for the resident and takes action to reduce a risk situation.

 

3.5     Demonstrates organization and time management skills as indicated by completion of routine assignments within established timeframes.

 

3.6     Performs routine technical skills consistent with policies and procedures.

 

3.7     Administers medications and treatments to residents per physician orders.  Monitors effectiveness.

 

3.8     Receives/interprets/implements physician orders within scope of practice.

 

4.                    Develops leadership skills that demonstrate commitment to the residents and Villa St. Francis.

 

            Delegates resident care activities to support personnel according to their level of knowledge and expertise.

 

            Seeks assistance in conflict resolution through interaction with immediate supervisor.

 

            Demonstrates knowledge of current policies and procedures or seeks resources if uncertain.

 

            Participates in the orientation of new personnel as opportunity arises.

 

            Assumes responsibility for availability of supplies and proper functioning of equipment necessary to provide resident care.

 

            In the absence of the Administrator, Assistant Administrator or Registered Nurse, functions as a charge person and accepts responsibility and accountability for all resident activities.

 

4.7   Participates in assessment process for potential residents in the absence of the registered nurse.

 

4.8   Updates and/or implements individualized service plans  for residents in need.

 

4.9   Assists the registered nurse with doctor visits (psychiatrist, podiatrist), inventor and supply maintenance, quality assurance reports and other delegated duties.

 

5.                    Develops skills that demonstrate commitment to personal and professional growth.

 

            Participates and attends departmental meetings, in-service, education programs and is responsible for accumulating appropriate hours of education to maintain competency.

 

            Recognizes stress in self and utilizes positive coping techniques.

 

            Completes plan for continuous improvement established at last feedback meeting.
