Our goal is to better the quality of life for those we serve while optimizing the allocation of community resources. Currently we serve frail elderly individuals and adults with disabilities living in Milwaukee, Racine, Kenosha, and Manitowoc Counties and have recently expanded our program operations in order to serve four additional southeastern Wisconsin counties. Current Opportunities include: Grievance and Appeals Coordinator ? Coordinates and facilitates the resolution of grievances and appeals initiated by or on behalf of Community Care Program participants/members. Internal Auditor ? Develops and performs audits and assesses the adequacy and effectiveness of Community Care?s system of controls over operations, financial reporting and compliance. LongTerm Care Administrator ? Develops innovative systems to maintain cost effective long term care networks that offer adequate service access and provide high quality care, and that facilitate positive working relationships with providers. PACE/Partnership Administrator ? Oversees the management and operation of PACE and Partnership Program operations in assigned counties. Monitors the quality of care and services being provided to Community Care program participants. Ensure?s compliance with all governing regulations and agency policies and procedures. Provider Data Base Specialist ? Responsible for ensuring that all provider information required by agency policy, or data necessary to analyze, plan and implement strategies to ensure Community Care?s ability to manage provider networks, is obtained, verified, recorded and maintained in the provider database.
