RN Case Manager

RN Case Manager Department: Case Management Schedule: Pool Position Shift: 1st shift Hours: Job Details: The scope of practice includes patient/family assessment and management of the assigned patient populations, multidisciplinary team leadership, program development, clinical outcomes measurement, clinical and fiscal variance analysis. The Case Manager leads Care Coordination Rounds and collaborates with all members of the multidisciplinary team to assure a coordinated plan of care for the patient and their families throughout the episode of illness and across the continuum of care settings.

EDUCATION/CERTIFICATION/LICENSURE: • Registered Nurse, licensed in the state of Wisconsin • BSN degree preferred or actively working towards BSN EXPERIENCE/SKILLS: • Minimum of 5 years broad clinical experience, predominately in medical/surgical nursing. Case Management, discharge planning and utilization review experience preferred. • Ability t o work independently, exercise independent judgment, establish priorities, and meet deadlines • Ability to establish and maintain positive, effective, collaborative relationships with patients, families, physicians, hospital staff and external providers • Demonstrates leadership skills and serve as a role model for clinical staff • Ability for complex clinical decision making and proactive creative problem solving • Ability to communicate well, verbally and written, with a positive attitude and commitment to customer service and excellence
