The provision of these services is socially significant and is intended to improve the quality of life while offering an alternative to nursing home placement. The purpose of the telephone interview is to determine if the applicant meets nursing home level of care criteria. Incumbents initiate and complete the LOCET tool or other assessment tools designed by the Department of Health and Hospitals. Excellent interviewing, evaluation, and computer skills are required in order to accurately complete the assessment. Computer input speed and accuracy are critical for completion of job duties.Incumbents gather existing evaluative and eligibility materials from various sources to verify the level of care needed and severity of medical condition, and verify recipient’s disability/diagnosis. Incumbents are responsible for all other duties as assigned.Education and Typical Years of Experience: Bachelor’s degree in health or human services, social work or related discipline with a minimum of three years of health related social and/or human service experience; at least one year experience working with mentally challenged or physically disabled persons A Master’s degree in Social Welfare or other related field may be substituted for two years of experience.Knowledge of healthcare, Medicaid, or Medicare is preferred. Specific Requirements: The work is very time sensitive. Applicants must be very organized and be able to meet stringent timelines. Work past the normal 8 hour day may be occasionally required. Computer literacy is a must. Applicants must have some demonstrable experience in Microsoft Word, Excel, and Outlook. Experience in conducting Needs Assessments is a plus.
