Qualifications -Need to utilize good reasoning skills to have an appropriate P.O.C. and to make adjustments as needs for changes in patients status. -Must be able to accurately complete billing, timesheets and tally evaluative tests. -Experience in evaluation/tx of adult population including disorders of swallowing, speech, neurological, hearing and voice. -Must be able to document clearly to meet reimbursement requirements. -Must have good speaking skills to be understood by residents. -Masters degree in Speech Language Pathology from accredited facility. -Possess a valid current state license in Speech Language Pathology or have a certificate of Clinical Competence from ASHA. Responsibilities 1. Evaluates, develops, and implements day to day Speech/Language Pathology program to ensure consistent delivery of quality resident care, and is an active member of resident care team. 2. Provide the delivery of SLP therapy in accordance with State and Federal licensure laws. Complies with Federal and State guidelines. 3. Ensures the resident care policies and procedures are followed, in compliance with applicable quality of standards. 4. Identifies resident care problems and concerns and contributes to short term and long term goal setting and approaches the interdisciplinary care plan. Informs and includes residents/and or families of their plan of care. 5. Ensures treatments are administered and documented in accordance with State regulations and facility policies and procedures. 6. Communicates significant changes in residents conditions to appropriate medical staff and rehab. team or designated individual. 7. Prepares and maintains accurately and timely patient documentation. 8. Orients staff as requested. 9. Attends all in-service education programs mandated by Rehab., Corporate, State and Federal guidelines. 10. Performs all duties in a safe and efficient manner and follows established guidelines when using physical restraints/protective devices and equipment. 11. Demonstrates awareness of potential hazards and reports unsafe conditions to Director of Rehab. and other staff when needed, to maintain a safe environment for residents, families, and staff. 13. Adheres to strict infection control guidelines as evidenced by appropriate use of personal protective equipment. 14. Ensures residents independence and right to make personal choices to accommodate their needs in accordance with the residents interdisciplinary care plan.
