Company Description Elite medical services company Job Description As the Case Manager, you will: Coordinate the care of patients with special high risk needs Contact the patient, patient's family, physicians, and other providers to ensure that the patient's health care needs meet established clinical quality standards Collaborate with other health care professionals to effectively evaluate and meet the needs of the high-risk patient Directly supervise the high risk coordinator Participate in clinical rounds with the inpatient case managers and medical director Utilize independent judgment, based on established professional standards of nursing practice, to plan, prioritize, and organize the clinical services of this population Assess the pathway of care for a patient based on their diagnosis Participate in and direct weekly high risk rounds in collaboration with medical director and high risk team Participate in the development and ongoing management of disease specific case management programs Review readmission reports and initiate appropriate interventions Create reports for tracking and trending outcome data Participate in utilization management and quality improvement committees Participate in special projects Required Skills 2+ years of acute care experience Case management experience in a managed care setting Familiarity with asthma, cardiac, discharge planning, diabetes, and disease management Computer literacy Valid New York State RN licensure Bachelor's degree in Nursing (BSN) Desired Skills Master's degree in Nursing (MSN) Company: Execu/Search Group Job Title: Case Manager Job Category: Medical/Health Location: US-NY-Long Island Salary/Wage: 57,000.00 - 73,000.00 USD /year Reference Id: J37687
